Written complaint must be stated in clear and understandable way and must include:
- Patient’s contacts (name, phone number, email address or postal address)
- Patient’s profession, clear statement with whom and what sort of problems have been raisen and againts whom the complaint is filed
- Clear description of problem in free form

- Run-through of complaint
- Run-through of patient’s medical file
- Evaluation of complaint and our proposals to solve the situation
